Am I eligible for a California medical card?

California uses an open-ended qualifying-conditions framework - broader than almost any other state. Most patients qualify if they have a documented medical reason that cannabis may help.

Common Qualifying Conditions

Chronic pain, anxiety, PTSD, cancer symptoms, migraines, insomnia, and more. CA accepts conditions cannabis relieves.

Age & Residency Rules

Must be 18+ (younger with parental consent and a designated caregiver) and physically located in California during the consultation.

What You'll Need

Government-issued ID, proof of California residency, and any medical records that support your qualifying condition.

Which program is right for me?

California runs medical and recreational programs side-by-side. The right choice depends on how much you spend, what you need, and how you'll use cannabis.

Medical (with MMIC)

Best for regular patients

$55–$155 / year · evaluation + optional county fee

  • Exempt from 7.25% state sales tax
  • Exempt from local sales tax (1–3% saved)
  • 8 oz baseline possession (vs 1 oz)
  • Higher home cultivation limits possible
  • Designated caregiver allowed
  • Access at 18+ (or younger with caregiver)

Right for you if:

You spend $100+/month, manage a chronic condition, want to grow at home, or are 18–20.

Recreational

Casual or occasional use

$0 upfront · pay full retail tax on every purchase

  • Full state + local sales tax (~9–10%)
  • 1 oz flower / 8 g concentrate limit
  • 6 plant home cultivation cap
  • No caregiver system
  • 21+ only, no exceptions
  • Same workplace protections (SB 700)

Right for you if:

Occasional use, under $50/month spend, no specific medical need, and you're already 21+.

Common cross-topic questions

The questions patients ask after they've read multiple guides - the ones that span topics.

Should I get my recommendation first or apply for the MMIC first?

The recommendation must come first — it's the prerequisite the county requires before issuing an MMIC. Standard sequence: complete an evaluation and get the physician's recommendation, then submit it to your county for the MMIC. You can shop at dispensaries with just the recommendation while the MMIC is being processed.

I'm moving to California with an out-of-state card. Do I start over?

Yes. Out-of-state medical cards aren't transferable. You need a California-licensed physician's recommendation and proof of California residency. Some California dispensaries will accept out-of-state cards on a recreational basis (paying full retail tax), but for medical benefits including the tax exemption, a California card is required.

I currently buy recreationally. What changes if I switch?

Three things change immediately: you stop paying state and local sales tax (the 15% cannabis excise tax still applies); your possession limit increases from 1 oz to 8 oz baseline; and your home growing limit can exceed 6 plants if your physician supports it. Most regular recreational users break even on the cost of an MMIC within 2–4 months.

If I get the recommendation, do I have to get the MMIC too?

No. The MMIC is optional. The physician's recommendation alone gets you medical access at any California dispensary. The MMIC adds the sales tax exemption and provides a state-issued ID card. For occasional users, the recommendation alone is often enough; for regular users, the MMIC pays for itself in tax savings.

What's the right order: card → dispensary → home growing?

Most patients follow this sequence: (1) complete the evaluation and get the recommendation, (2) optionally apply for the MMIC, (3) visit a licensed dispensary for first-purchase guidance, (4) explore home cultivation only after you understand product types and dosing through dispensary purchases.

How does California compare to other state programs?

California is uniquely open. Most states use closed lists of qualifying conditions; California allows physicians to determine eligibility for any condition where cannabis may help. California also has the lowest age threshold (18+ with full eligibility) and one of the most permissive home cultivation frameworks.
