Illinois Medical vs. Adult-Use Cannabis

Illinois offers both medical and adult-use access, giving eligible patients more than one option. Compare the benefits, costs, and legal protections to determine which path best fits your needs.

Illinois Medical Cannabis Program

Ideal for eligible patients

  • Pay just the 1% qualifying-drug tax rate instead of the recreational excise tax
  • Buy and possess more per visit than recreational shoppers (2.5 oz per 14 days vs. 30g on hand)
  • Grow at home if you're 21+ and registered - up to 5 plants
  • Protected from certain employment and housing discrimination
  • Requires a physician certification and IDPH registration

Recreational Marijuana

  • No physician visit or IDPH registration required
  • Walk into any licensed adult-use dispensary if you're 21+
  • Pay the full stacked tax - 10-25% cannabis excise tax plus state and local sales tax
  • Lower possession limits than medical cardholders (30g flower / 5g concentrate / 500mg THC edibles)
  • No legal home cultivation - growing without a medical card remains prohibited statewide

Curious What the Tax Difference Actually Adds Up To?

Medical patients pay roughly 1% state tax on cannabis purchases; recreational buyers pay a stacked 20-41% depending on product type and city. Run the numbers and see how much a medical card could save you over a year of regular purchases.
