When a licensed healthcare provider certifies you, you receive a patient certification. That certification acts as your access document. You show it at a licensed dispensary alongside a government-issued photo ID, and that is enough to purchase. You do not need to bring anything else. No separate card, no registry printout.

Your certification has an expiration date set by your provider. The provider uses professional judgment to determine the appropriate length of treatment, and they can issue the certification for a period shorter than a year if they feel that is clinically appropriate. Most doctor certifications are valid for 2 years. Check the date printed on your certification so you are not caught off guard.

New York updated its medical cannabis program, and some of these changes directly affect patients going through renewal.

Governor Hochul signed Senate Bill 2025-S3294A in November 2025, representing one of the most significant updates to New York's medical cannabis framework since legalization expanded statewide. Among the changes, the legislation streamlined the patient certification process and reduced participation barriers across the state.

The renewal process follows the same basic path as your initial certification. There is no complicated re-registration, and there is no state fee to worry about.

Here is what the process looks like:

Step 1: Check your expiration date

Log in to the OCM patient portal or look at your current certification document. Give yourself at least a few weeks before the expiration date to avoid any gap in access.

Step 2: Book an appointment with a certifying provider

You can do this in-person or via telehealth. If your condition has not changed significantly, a telehealth appointment is typically straightforward. You can search for certified providers through the New York State Office of Cannabis Management's official provider directory.

Step 3: Complete the consultation

Your provider will review your condition and determine whether continued certification is appropriate. Be honest about your symptoms, what is working, and what is not. This is a real medical conversation.

Step 4: Receive your updated certification

Once your provider certifies you, your new certification can be used immediately at any licensed medical dispensary in New York State, either as a paper copy or displayed on a smart device.
