How to Register as a Proxy Applicant?
After creating a New York government account on a minor patient or physically or mentally incapable patient’s behalf and activating it, you will require the below-mentioned steps to register for the medical marijuana card New York Online:
STEP 1
After signing in to the official portal of DOH, click on the Health Applications icon and then select Medical Marijuana Data Management System (MMDMS).
STEP 2
Now, select the “proxy applicant” option that will land you on the Medical Marijuana Program for registration.
STEP 3
Now submit the patient’s information, including:
- Full Name of the patient
- Date Of Birth of the patient
- Patient Certification number issued by a licensed physician
Note:
- You can find the certification number in the top right-hand corner of the recommendation. Enter the number as written on the certification but don’t add spaces. Click here to apply for the MMJ certification!
- If you see an error message on the screen, recheck your entered information with the certification. If you get the same message, email the Department of Health at mmp[at]health.ny.gov to update the information.
STEP 4
Enter the following information and click on the “Next” button:
- First Name
- Last Name
- Date of birth (The format should be MM/DD/YYYY)
Note: If you are applying as a proxy applicant, you must add the first name and last name as they appear on Department of Motor Vehicles ID or New York State Non-Driver ID. Do not enter your middle name while applying.
STEP 5
Now click on the “REGISTER/VIEW REGISTRATION” button.
STEP 6
After clicking on the “REGISTER/VIEW REGISTRATION” button, you will need to validate the patient’s information.
Note: The certification information of the patient can not be changed. If you have found any mistake in the registration process, please email the Department at mmp[at]health.ny.gov for correction if you find any incorrect information.
STEP 7
After validating the patient’s information, enter the remaining details of the patient, including:
- NYS DMV ID or NYS Non-DriverID Card of Patient
- Proof of Patient Residency
Note: If you need to update a patient’s mailing address, you must upload the supporting documents. Click here to check out the valid documentation of adult and minor patients.
STEP 8
Enter your contact information so that the Department will be able to contact you when needed.
Note: Keep in mind that
- If you have already set your relationship as Parent/Legal Guardian, the bottom “Parent/Legal Guardian Information” section will not be shown.
- If your entered information is incorrect, please email mmp[at]health.ny.gov for updates.
STEP 9
As a proxy applicant, you must add at least one caregiver to the patient registration. If you are also designated as a caregiver for the patient, then click on the “Add me as Caregiver” button.
STEP 10
You can also add a different caregiver or more than one caregiver. Click the “Add Caregiver” button. Enter the following information, then click on Add button:
- Full name
- Date of birth (The format should be MM/DD/YYYY)
- Zipcode
- New York State Department of Motor Vehicles (NYS DMV) ID
Note: The caregiver should add the NYS DMV ID with the New York State Non- Driver ID number or the Driver’s License number.
STEP 11
Once the application is complete, check out the acknowledgment statement. Then, answer the question regarding Driver’s Licenses and Non-Driver ID Cards.
Note: If the photo shown on the ID card no longer resembles you, you need to upload a recent photo.
STEP 12
You need to select the “Submit Registration” and then the “Save & Exit” button for the final submission of your application.
Registration Status
After submitting your registration, you will see a pop-up message on the screen that shows your registration status within a few minutes.
- Pending Review: Your enrollment will have pending status if you upload certificates or a photo. The Department will evaluate your application to verify. You can log back into your account to check the status of your enrollment at any time.
- Approved: If the status is shown as approved, it means you can download your temporary registry ID card that can be used to buy medical marijuana from a registered organization’s dispensing facility until you get your valid marijuana card in New York online.
