Becoming an Arizona Designated Caregiver is straightforward. Start by filling out an online application. You need to be at least 21 years old, a resident of Arizona, and have no felony convictions. The patient you’ll be caring for must be certified by the Arizona Department of Health Services (ADHS).

Refer to the ADHS online checklist for required documents, including the patient’s application ID number. Complete the Medical Marijuana Caregiver Attestation Form and submit fingerprints for a background check.

A custodial parent or legal guardian is required for patients under 18. A caregiver can assist up to five patients. The caregiver ID card costs $150 and must be renewed annually. Submit your renewal application at least 30 days before the card expires. Application fees are non-refundable. ADHS will notify you via email if your application is accepted or needs resubmission.
