In Oregon, patients must have been diagnosed with certain qualifying medical conditions recommended by a licensed healthcare professional for Oregon medical marijuana card. The following medical conditions may qualify you for an Oregno medical marijuana card:

• AIDS/HIV
• Cachexia
• Cancer
• Chronic or severe pain
• A degenerative or pervasive neurological condition
• Glaucoma
• Persistent muscle spasms, including multiple sclerosis (MS)
• PTSD (Post-Traumatic Stress Disorder)
• Seizures including epilepsy
• Severe nausea
• Severe pain

• Marijuana recommendation statement from the licensed healthcare professional.
• Grow Site Consent Form to notify the department where a grow site is designated and the patient is not the property owner.
• Copies of current valid photo ID

  4. Military identification card;
  5. Tribal photographic identification
• Minor Declaration Form if applying for a minor.

You can use a Valid driver’s License, State ID Card, or US Passport to prove your residency and Identification.

Patients may designate caregivers to help them obtain, use, or cultivate cannabis. Patients younger than 18 must assign a caregiver, and their caregiver must be their parents or legal guardian. To qualify as a caregiver, you must be

• At least 21 years old.
• Oregon residents with valid state or federal identification.
• Free of any criminal charges related to substance abuse.

OMMP does not allow a patient to have more than one caregiver. However, the program did not specify how many patients a caregiver can manage. Designated growers can grow up to 6 mature marijuana plants, 12 immature plants 24 inches or more, and 36 immature plants below 24 inches in height for each patient under them.
